If you are having a medical emergency, call 911. If you are an established patient of this practice and require urgent medical assistance after hours or on the weekends, please call our office at

We require our patients to be seen at the requested intervals in order to refill medications. This allows us to provide the best care to our patients and assess if the medications are still working appropriately, assess for any adverse side-effects, and determine if the medication is still appropriate for your condition. Some medications or treatments may require more frequent follow-ups; your provider will advise when a follow-up appointment is required. To request a refill, notify your pharmacy first, who will then contact us. Refill requests will take up to 72-hours to process.
Kindly provide over 24 hours’ notice if you need to cancel an appointment. While we understand circumstances arise that may require you to cancel your/your child’s appointment at short notice, to better serve all our patients there is a late cancellation fee of $50. This fee applies to no-show appointments or late cancellations (less than 24 hours notice). This fee will not be covered by insurance. If you need to cancel an appointment, please call us at .
We are happy to assist with the completion of school forms, etc. for patient’s allergy/asthma medications, epi-pens, and the like. Forms will be reviewed, signed by the provider and ready for pick-up in one (1) week. There is no charge if school forms are brought in during a scheduled office visit. For school forms brought in outside of a scheduled office visit, the fee is $10 for the completion of each page on a school form. Payment is required in advance if utilizing this service.
